In accordance with New York state education law, any student participating on an organized athletic team must have an annual physical to participate. Physicals are good for a period of one year from the date of the physical. Athletic Health History Forms must be completed for each sport season. The form is then submitted to the school nurse to be completed. Students must have a physical and/or update in order to participate.
The health office encourages all athletes to see their own primary care provider, as the exam is more comprehensive. However, should an athlete not be able to see their own doctor, a representative from the school medical director’s office will be at the high school to perform sports physicals on:
- Tuesday, July 31, from 4-7 p.m. at the High School
- Friday August 10, from 9 a.m.-12 p.m. at the Middle School
Health update and health examination forms can be dropped off at your students school in the main office. Please note the health update forms can be turned in no sooner than 30 days prior to the start of your student’s sport. If you have questions regarding your student’s medical clearance, please contact the health office in your student athletes building for more information.
The following options are available if your son or daughter wants to participate on an athletic team:
- Obtain a physical at the school. Physicals are done by the district chief medical officer.
- Your child may get a sport physical by their private physician. The physical MUST be completed on the Health Appraisal physical form and state the student is cleared to participate in athletics. Completed forms need to be returned to the school nurse. If you choose to have the physical done by your private physician, the completed physical form MUST be delivered prior to the start of practice to the school nurse or athletic office.
