RCS Interscholastic Athletics Medical Clearance Information and Process

Student-Athlete and Parent Responsibilities—

1. All potential student-athletes must have an up-to-date sports physical.The form must be filled out and signed by the doctor and also signed by both the parents. The forms get turned into the nurse in the health office.

The health office encourages all athletes to see their own primary care provider, as the exam is more comprehensive. However, should an athlete not be able to see their own doctor, a representative from the school medical director’s office will be available on-site, click here for dates.

Health update and health examination forms can be dropped off at your students school in the main office. Please note the health update forms can be turned in no sooner than 30 days prior to the start of your student’s sport. If you have questions regarding your student’s medical clearance, please contact the health office in your student athletes building for more information.

2. Student-Athletes must sign-up with their coach or in the health office with the nurse.

3. Within 30 days of the start of any season, (Fall-Winter-Spring) the student-athlete must turn in a health update form (available online) IN PERSON to the nurse. The nurse will tell the student-athlete if they are cleared or send them off with the required information for clearance. ALL STUDENT-ATHLETES MUST BE “MEDICALLY CLEARED TO PLAY” by the nurse prior to the start of practice. A student-athlete that has not been cleared is ineligible to participate, and practices will not count until the athlete has been cleared by the school nurse. If you cannot attend either of these two days, you must contact the Health Office and make an appointment to bring the update in and have the student-athlete cleared. There will be limited availability due to summer hours.

4. Each coach and the athletic director will receive a list of athletes who have been “cleared to play.” Only athletes on this list will be allowed to participate.
